Ankylosing spondylitis and other spondyloarthropathies, Antiphospholipid syndrome, Arthritis, Autoinflammatory diseases, Behcet's disease, Bursitis, Carpal tunnel syndrome (CTS), Gout and pseudogout (also called calcium pyrophosphate deposition/CPPD), Granulomatosis with polyangiitis (Wegener’s), Giant Cell Arteritis ( GCA),

Inflammatory myopathies, Lupus (systemic lupus erythematosus), Osteoarthritis, Osteoporosis, Palindromic rheumatism,Polyarteritis nodosum, Polymyalgia Rheumatica (PMR), Polymyositis or dermatomyositis, Psoriatic arthritis, Raynaud's disease, Rheumatoid arthritis, Sarcoidosis,Scleroderma, Sjogren's syndrome, Tendinitis,Vasculitis

IV Infusions

We offer in-office infusion for rheumatology medications, with a board-certified provider on-site during our infusions. 

This allows our IV nursing team to collaborate with the providers and ensure you are receiving the best care and treatment possible.

 will keep you on the path to meeting them.

Appointments

We are accepting new patients; please contact your insurance to verify coverage and see if a referral or prior authorization is needed for a Rheumatologist to provide care and treatment for you. 

Appointments are Available at Our Kihei office every other weekend. 

Patients are expected to bring their insurance card and photo ID to every appointment.



Due to the high demand for Rheumatology, appointments are canceled without at least 24-hour notice, and those who do not show up for their appointment will be charged $150. 

This fee will not be billed to your insurance but will be billed directly to you, the patient. 



Referral

We require a referral with recent chart notes and labs from your primary care physician, specialist or current rheumatologist to schedule an appointment; 

please request that these be faxed to 866 397- 2741.


We do not accept self-referrals at this time.

We ask that you contact your insurance directly for your benefits and coverage.

We will require a pre-authorization from your insurance company 

for all out-of-network plans. 


We do not accept the following insurance even out of network:
